Participation Agreement, Release and Waiver of Liability

Please read this agreement carefully. By signing it you are agreeing on behalf of yourself and the participant named above.
1. Voluntary participation
I am the parent, legal guardian, or the adult participant named in this registration, and I am authorised to sign this agreement. I understand that participation in ITeach Inc. programs — including the Soccer Academy, the Computer Academy, the after-school program, practices, matches, classes, trips and events — is entirely voluntary and offered at no cost.
2. Assumption of risk
I understand that athletic activity involves inherent risks, including running, physical contact, falls, contact with equipment or other participants, weather conditions, and travel to and from activities. These risks can result in injury ranging from minor to serious, and in rare cases death. I accept these risks knowingly and voluntarily.
3. Release of liability
In consideration of the participant being permitted to take part, I release and hold harmless ITeach Inc., its directors, officers, employees, coaches, instructors, volunteers and facility hosts from any claim, demand, or cause of action arising out of ordinary negligence in connection with the participant's involvement in the program. This release does not apply to gross negligence, recklessness, or intentional misconduct, and it does not waive any right that cannot be waived under Kentucky law.
4. Authorisation for emergency care
If the participant is injured or becomes unwell and I cannot be reached, I authorise ITeach staff to seek and consent to emergency medical treatment on the participant's behalf, including transport by ambulance. I accept responsibility for the cost of that care. I confirm that the emergency contact and care information I have provided is accurate and current, and I will tell ITeach promptly if it changes.
5. Care information
Any information I provide in the care notes field is shared with coaches and instructors only so that they can supervise the participant safely, and is kept no longer than needed for that purpose. Providing it is optional.
6. Equipment and property
Uniforms, laptops, tablets and other equipment loaned to the participant remain the property of ITeach Inc. I agree that they will be returned in reasonable condition at the end of the season or course, and that they will be used for program purposes.
7. Photography and media
Photo and video consent is optional and is recorded separately below. If I grant it, I agree that ITeach may use photographs and video of the participant in program materials, on iteachky.org, in grant reports and on ITeach social media, without payment. Consent may be withdrawn at any time in writing to support@iteachky.org, and ITeach will stop using new images and remove existing ones from its own channels where it reasonably can.
8. Behaviour and dismissal
I have read the code of conduct below and agree that the participant and accompanying family members will follow it. ITeach may suspend or dismiss a participant whose conduct endangers or repeatedly disrupts others, and will explain the reason when it does.
9. Communication
ITeach may contact me at the phone number and email address provided about schedules, closures, cancellations and program matters. Newsletter consent is separate and optional, and I can unsubscribe at any time.
10. Whole agreement
This agreement is governed by the laws of the Commonwealth of Kentucky. If any part of it is found unenforceable, the rest remains in effect. I have read it, I understand it, and I am signing it freely.
Player, student and family code of conduct
- Show up on time, and let a coach or instructor know when you cannot make it.
- Treat teammates, opponents, referees, instructors and volunteers with respect — on the pitch, in the classroom and online.
- No bullying, harassment, slurs, or fighting. This applies to family members on the sideline too.
- Take care of the equipment, the room and the field you are given.
- Phones away during class and practice, unless an instructor asks you to use one.
- Ask for help early. Nobody here expects you to already know it.
